昨日の午後のオペは、上の顎に７本のインプラントを
埋入するオペでした。
この患者様も骨が無いので、骨を足す必要があったのですが
最近ほとんどのケースでは、ソケットリフトという方法で
骨を足します。今回、この患者様はサイナンスリフトという
方法で骨を足しました。
どちらも、骨を足す方法なのですが、
ソケットリフトは、インプラントを入れるための最小限に
削った穴から上顎洞に骨を足す方法で、患者様の負担も
少なくて済むため、最近は、ほとんどのケースでこの方法を使用します。
本日の患者さんのように、どうしてもそれでは対応
出来ない方に限り、ほっぺたの横から、大きな穴を開けて
骨を足す方法で、一般的によく言われるサイナンスリフトと
いう方法を用います。
オペをする私としては、良く見えるのでやりやすい方法ですが
患者さんの負担が大きいため、あまり使用しません。
ですので、部分的（副腔部分）にはソケットリフトで済まし、どうしても、
必要なところ（４本）のみ左右ともサイナンスリフトとし、インプラントを
埋入しました。
患者様には、腫れることを事前にしっかり説明していましたが、
たった今、消毒にこられ、全く腫れもなく痛みもないとのことでした。
決して、腫れたから失敗という訳ではないのですが、
やはり患者様の負担を考えると、腫れがないとほっとします。
昨日も２つのオペをしたのですが、午前中に行った下顎の２本
埋入の方の方が腫れていたりして・・・。
